Reports and Forms
If you would like to make any anonymous report or complete any of the forms that follows with the Discord server or SMP, this is the following forms we currently have open.
Content Creator spots are limited to 10 people. We are looking for active creators who are genuinely interested in playing, streaming, and helping bring cozy, fun energy to the server. Being accepted means you are expected to stay respectful, follow all rules, and create content when you are able.
This form is here so you can share ideas, feedback, or concerns to help improve our cozy gaming community. Whether it’s a feature you’d love to see, a change you think would help, or something that hasn’t been working well, we truly want to hear it.
That said, please be as detailed and specific as possible. Anonymous suggestions only help if we can understand exactly what’s going on. If your message is vague, missing context, or doesn’t explain the issue clearly, we won’t be able to act on it.
This form is for reporting rule violations, inappropriate behavior, or situations that require staff attention. All reports are taken seriously and reviewed carefully.

If you are unable or unwilling to include names or describe the issue clearly, we won’t be able to help, as vague reports give us nothing to act on.
